CheckUp America Diabetes Test is a mail-in fingerstick test for diabetes, brought to you by the American Diabetes Association in cooperation with Home Access Health Corporation. This product allows an individual to self-test for diabetes from the comfort and privacy of their home.

The CheckUp America Mail-in Lab Diabetes Test includes:

• Hemoglobin A1c

This test is the same testing method used by Doctors and Hospitals throughout the world. One difference: it only takes a fingerstick sample of blood.

It is estimated that 20 million Americans have diabetes and 6 million of these are completely unaware they are diabetic. Alarmingly, the American Diabetes Association) estimates that over 40 million Americans between the ages of 40 and 74 have pre-diabetes and are at high risk for developing diabetes.

This product is currently available under the direction of a physician order. Home Access has developed a relationship with a National Physician Network to order and review testing at no additional cost, for individuals purchasing this product.

The CheckUp America Diabetes Test includes a collection kit, laboratory test services, and support from our medically directed contact center. Each kit includes everything necessary to collect, package and mail a small finger-stick sample to our laboratory using a prepaid, pre-addressed mailer. All testing is performed in a CLIA-regulated, CAP-certified laboratory.
